When something is actively going wrong, nobody is going to fill in a form well. This agent runs the capture as a conversation that works under pressure: it asks for what a regulator will later want, refuses to silently skip a field, and writes down every assumption as an assumption.

Capture, while it is still happening
A coaching conversation rather than a form: when the answer is not known yet, that gets recorded as not known yet rather than left blank or guessed. Detection time, affected systems, severity and notification obligations are all asked for explicitly.

An after-action pass once the pressure is off, when people remember more and are willing to say what actually went wrong. This is where the useful material comes from, and it is the step most processes skip.

The security officer reviews, corrects and signs. The agent transcribes that decision; it never makes one. An incident record where a person's judgement and a machine's suggestion look alike is worth much less as evidence.
